Because we don't quite have online registration ready for the Fall 2008 yet, we've provided a simple form to collect the list of girls who will be coming to tryouts. It's very important that everyone try to register beforehand so we can balance the tryouts properly. If for some reason you absolutely cannot register beforehand, then please show up at one of the tryout times anyway, but we can only accommodate a very small number of "walk-ons", and you may have to wait until the next session. (If you walk-on for the 5PM session, you're taking a chance.)

NOTE -- BY ADDING YOUR DAUGHTER'S NAME TO ANY OF THE LISTS BELOW, YOU ARE AGREEING TO THIS WAIVER RELEASE: I, the parent/guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the USYSA, MYSA, BAYS and NGS, their affiliated organizations and sponsors. I desire to have the registrant participate in the soccer programs and activities, including indoor/outdoor play, practices, clinics and matches (“Programs”) offered by USYSA, MYSA, BAYS, NGS, and/or their affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with soccer and the registrant’s participation in the Programs, and in consideration for the USYSA, MYSA, BAYS and NGS accepting the registrant for participation in the Programs, on behalf of myself and the registrant, I hereby release, discharge and/or otherwise indemnify the USYSA, MYSA, BAYS, NGS and their affiliated organizations and sponsors, their respective officers, directors, coaches, committees, employees and associated personnel, including the owner of fields and facilities utilized for the Programs, of and from any claim, demand, action, cause of action, suit or liability arising as a result of the registrant’s participation in the Programs, including the transport of the registrant to or from the Programs, which transportation I hereby authorize. EMERGENCY MEDICAL CARE: As parent or legal guardian of the minor named on this form, I hereby give my consent to seek, obtain and provide emergency medical/dental treatment in case of injury that occurs while participating in NGS-related activities. This care may be given under whatever conditions are necessary to preserve life, limb or well-being of such minor. I understand that such treatment will be sought and provided only in an emergency and that reasonable efforts will be made to contact me before providing such treatment. Player Safety: No jewelry of any kind may be worn during play--no bracelets, necklaces, earrings or rings. Players may not wear beads, barrettes or other hard/sharp items. Newly pierced ears may not be taped. Players may not participate with casts or splints of any kind. Shinguards must be completely covered by socks at all times. NO EXCEPTIONS! |
The assessment is only held on one day -- Saturday, May 10. If you are unable to attend assessments that day, please do not be concerned. Your team placement will be based on the evaluation of your current coach, which is in any event the more important criteria. It is not necessary to notify us beforehand that you cannot attend the assessment. (If you cannot attend the assessment, and are new to NGS, and therefore won't have an assessment from a previous coach, please do contact the Travel Committee and we'll try to arrange some means of assessing you.)

Usually, your daughter should be entering the 7th or 8th grade this Fall to play in

The U14 assessments will take place on Saturday, May 10 (weather permitting)
on the Newton South fields.
Session times are either 3:00PM, 4:15PM, or 5:30PM.
The U12 assessment was held on Saturday, May 3 2008. If you missed it, don't worry, you may still sign up to play U12 soccer this Fall, as soon as the "full" registration (with payment) is available.
The U10 assessment was held on Sunday, May 4, 2008. If you missed it, don't worry, you may still sign up to play U10 soccer this Fall, as soon as the "full" registration (with payment) is available.
