You must have Adobe Acrobat installed to view forms in the PDF format.

Even with Acrobat installed, some versions of Microsoft Internet Explorer (IE) on Windows have been known to display a blank page instead of the downloaded form.  If you experience this problem, you may need to install an update to IE -- either a whole new version or a service pack.  If you are running IE 5.01 SP2, 5.5 SP1, or IE6 you should not have this problem.  Use Help About in IE to find out what version of IE you have.  If you are having a problem viewing PDF files and you are running an older version of IE, you should consider installing an update.

Note if you use AOL versions 4 through 6, then you're effectively using IE.  To find out what version of IE you have, start up IE (not the AOL client -- it doesn't matter if you're logged in or not), and do Help About.
